As a provider of health and human services, you’ve seen the phrase “Continuum of Care” used in various ways to offer a vision for the future. And what a wonderful future it will be for providers and the individuals they serve. A pre-session survey of behavioral health leaders indicated that the majority believe that it is somewhat or very easy to understand where individuals are within their organization’s continuum of care. Does that match your experience?
Patty Knox and Sue Jacoby from Cordant Health Solutions and Murray Heidebrecht and Scott Borisoff from EnSoftek presented “Journey to the Modern Continuum of Care” at the Oregon Council for Behavioral Health (OCBH) July Full Member Meeting. The session breaks down this future vision into steps that organizations can use to optimize the care offered within their walls and lay the foundation for participation in a care continuum that serves individuals across providers.
A pre-session survey was completed by 24 of the attendees. The data gathered from this survey is shared during the OCBH presentation and offers insight into where the attendee organizations are in their journeys toward a modern continuum of care.
